Clinical Letters & Medical Documentation — $150

Sometimes recovery needs paperwork: a letter supporting workplace accommodations, school supports, medical leave, or treatment continuity. Mindful Sewa Psychiatry provides clinical letters and medical documentation for established patients when clinically appropriate, at a flat $150.

Every letter is written carefully, accurately, and on professional letterhead by Harpal Singh, PMHNP-BC.

Documentation we provide

Workplace accommodation letters (ADA), school and testing accommodations (504/IEP support), FMLA and medical leave support, treatment summaries for care coordination, and continuity-of-care letters when transferring providers.

How it works

Documentation requests follow an evaluation or are made during active treatment, because honest documentation requires genuine clinical knowledge of you. Most letters are completed within 3–5 business days of the request.

What we do not provide

Letters require clinical justification. We do not provide documentation that is not supported by evaluation findings — that protects the credibility of every letter we write, including yours.
